It is in the "Rich Text" format that can be opened with MS Word. After filling in the information, please save your file as either a "Rich Text Format" file or a Word file and email it to membership@casn-online.org.
Trainee members do not need to pay membership due. For regular members, please also send your payment (Money Orders in US funds or Personal Checks from US banks, payable to Chinese American Society of Nephrology; in the amount of US dollar $50.00 for annual membership or $350.00 for lifetime membership), along with a printout of the membership application and renewal form, to:
